How do I request patient transfers?

Call ACP and talk to our Patient Intake Team. We’ll guide you through the process that involves meeting with your family, conducting a clinical assessment, and gathering the necessary paperwork. What are the next steps after I call ACP?

ACP will meet with you and do a clinical assessment, speak with your physician, and start gathering all medical paperwork that requires your signature. When it comes to patient transfers, we will even contact your current agency to let them know you are transferring so you don’t have to make that call. ACP prioritizes your comfort and continuity of care above all else. We ensure a seamless transition by meticulously organizing all the necessary documents and coordinating with all involved parties. How long do patient transfers take?

While every Medicaid program and insurance company is different, patient transfers typically take no longer than 30 days and, on average, are completed in just a couple of weeks. Many transfers happen on the first day of the following month after you call us. If you have any questions or concerns throughout the process, our staff is standing by to provide updates and guidance. To further facilitate your transition, we engage directly with your current provider to ensure all necessary information is transferred accurately and promptly. This collaborative approach helps us maintain the continuity of your care, ensuring that your medical needs and preferences are fully understood and respected by your new care team. Can patient transfers be denied?

No. By law, your transfer cannot be denied. Medicaid patients are required to sign a form called Freedom of Choice. This document states that you are not under contract and you have the right to leave a provider at any time for any reason. Understanding your rights is crucial during a transfer. What happens after my transfer is processed?

Once your transfer is processed, ACP will electronically receive a prior authorization giving us permission to take over care. While we are waiting for this prior authorization to arrive, our scheduling team will be in touch to discuss scheduling needs as well as what you are looking for in a caregiver. Rest assured, our nurses are trained in caring for your family member well before they show up at your home. Additionally, to ensure a personalized and comfortable care experience, we encourage open communication about your expectations and any specific requests you may have. This initial dialogue is vital for tailoring our services to fit your family’s unique situation and preferences, setting the foundation for a trusting and effective caregiver-patient relationship. I’ve officially transferred. What’s next?

Depending on your insurance or waiver program, services can either start immediately or on the first of the month. Your caregiver(s) will arrive based on the predetermined schedule that you and your Scheduler created, and they will provide care for your loved one as well as education for any unfamiliar caregiving procedures. Your Clinical Supervisor is your resource to answer any clinical questions you may have. To enhance your care experience, we closely monitor the effectiveness of the care plan and remain flexible to adjustments. This adaptive approach ensures that your loved one’s needs are met with the utmost precision and compassion. We will need the patient’s full name, DOB, Medicaid ID Number, name and point of contact for your current company, and reason for transfer. We will also require a digital signature from a parent/guardian before submission.

Can family members accompany the patient during the transfer?

Family members may accompany the patient, subject to safety and health regulations. Prior coordination with our team is necessary to ensure accommodations meet all parties’ needs and compliance with health standards, ensuring a supportive transfer experience.

How long does the transfer process take from initiation to completion?
