Skilled Nursing Care
We offer private duty nursing for children and adults through the GAPP Program, COMP/NOW Waiver Programs, School Systems and other Insurances. We offer care for: ventilator dependencies, tracheostomies, IV therapies, tube feedings, wound care, colostomy care, neurological disorders, respiratory diseases/disorders, transplants, cancer, gastrointestinal disorders, endocrine diseases/disorders, IDD individuals, quadriplegic/paraplegic, cardiac defects and other complex needs.
Personal Care Support
Our CNAs (Certified Nursing Assistants) assist medically fragile children and adults with daily living activities such as bathing, dressing, meals and housekeeping around the clock or whenever you need it. Services include: assistance with daily living activities (bathing, meals and housekeeping), monitoring vital signs (including blood pressure, pulse, respiration, temperature), and assistance with ambulation, transfers and more.
When it’s time to go to school and our kids need extra help, ACP is here to help them get stronger and smarter. We partner with the school system to provide one-on-one nursing without interfering with your educational goals. We’re there every step of the day, from getting ready for school and riding the bus to being in the classroom.
Prescribed Pediatric Extended Care (PPEC) are medical day care centers that allow Medicaid eligible children from birth through age 20 with medically-complex conditions to receive continual medical care in a non-residential setting.
Such programs can dramatically improve patients’ functional status and social development while giving families a break from the day-to-day rigors of caring for chronically ill children up to 12 hours per day, 6 days per week. Children receive nursing, personal care and developmental therapy services.
Hospital to Home
The transition home with a medically fragile child or adult can be difficult at best, and we can help make this process smoother and more comfortable instead overwhelming. From coordinating the discharge process with the hospital case manager to settling in, from the first time you take your baby home or for a return visit, having the right help in place is essential.
