From finding the right caregiver to understanding your options, we know families with medically fragile children need a provider they can count on.

We Put Families First

We understand the unique and delicate situation your family finds itself in when it comes to raising a medically fragile child. You deserve a true partner that will stand by your side throughout your journey. Communication is the key to your family’s success — and ACP is here to facilitate that. From help with the GAPP approval process to finding the best caregiver that matches your family dynamics, we’re your partner.

Understanding Your Options

There are many ways you can qualify for skilled nursing care and/or personal care support services. And we are here to help you to qualify.

GAPP is a Medicaid program that provides services to medically fragile children up to 21 years old in need of skilled nursing care and personal care support. We can help you every step of the way.

Katie Beckett Waiver

The Katie Beckett Waiver is an eligibility category for Medicaid for children under the age of 18. It takes the parents’ income out of the picture and bases the decision on the child’s needs.

These Waivers offer home and community-based services for people with a developmental or intellectual disability. ACP provides Nursing and CLS Services to COMP/NOW Families

Private Insurance

Private insurance often covers in-home care. We can handle that entire process for you — from verification of benefits to prior authorization. We take the hassle out of your hands.

School Coverage

In many cases, families can get aid through school systems. We can help you understand how to determine what you qualify for and the actions to take to get it.

Private Pay

Families always have the option to take their care into their own hands with private pay. In this case, we work with families to establish a payment program that works for everyone.

The ACP Difference

The in-home care provider you choose has to make a huge positive difference in your family’s day-to-day life. ACP’s team is located in a single office — like your family, we like to have everyone in the same house so we can collaborate and communicate in the most efficient manner, and give you the responsiveness you deserve.

Every caregiver on our team is highly qualified, certified and has the right disposition to care for your loved ones.

We believe ongoing training and certificate maintenance shows our commitment to excellent care.
